For years, local search worked a certain way. You figured out the keywords people were searching for, you worked them into your page titles and your content, and if you did enough of that, you showed up. Keyword density was the game, and the question being asked was basically the whole strategy.

That is not how it works anymore. And if your website is still built around that old model, you are working against yourself.

The Shift Nobody Explained to You

Over the last four or five years, especially with the rise of AI search, what shows up in results has become far more about authority than about keywords. The subtle but important shift is this: instead of focusing on the question someone is asking, you need to focus on being the answer.

That distinction sounds small. It is not. It is the difference between showing up in local and AI search and getting passed over entirely.

The Three Things That Actually Determine Local Search

There are three categories worth understanding here: relevance, proximity, and prominence.

Relevance is the most basic. If you want to show up for chiropractor searches, you have to actually be a chiropractor, clearly and unmistakably communicating that to Google and AI. If you want to rank for a specific technique, like Gonstead or Upper Cervical, you have to actually practice it and say so. If you want to rank for pediatric chiropractic, you need to actually see kids.

Proximity means making sure Google and the people around you know exactly where you are. This part is genuinely basic, and yet some chiropractic websites, even ones built by high-volume website companies, do a surprisingly poor job of clearly establishing location. What counts as “close” also depends on where you practice. In a dense city, close might mean ten blocks. In a rural area, it might mean twenty miles.

Prominence is the hard one. This is where most chiropractors are leaving results on the table, and it has nothing to do with keyword density.

The Gas Station Chicken Wing Problem

Here is a way to think about prominence. Imagine a gas station that happens to sell chicken wings, versus a restaurant that is known specifically for chicken wings. The gas station could plaster the word “chicken wings” all over one page of their website, but nobody actually goes there for the wings. It is a secondary thing.

A lot of chiropractic websites function the same way. A page that mentions headaches once, or spinal decompression once, or pediatric chiropractic once, is the digital equivalent of that gas station. It technically says the word. It does not establish expertise.

The only situation where a thin mention is enough is if you are already a big deal. Someone like Michael Jordan does not need a deep, content-rich website to prove he is good at basketball. Everybody already knows. But most chiropractors are not already famous off their website. Most chiropractors are trying to build that authority in the first place, which means the website actually has to carry the weight.

What Building Real Authority Looks Like

If you want to become known as the headache doctor in your area, mentioning headaches once on a services page is not going to do it. What actually moves the needle is depth: a robust page about headaches, the different types, what works and what does not, paired with a real body of supporting content, blog posts that go deeper into specific aspects of the topic over time.

It also extends beyond your own website. Have you guest blogged somewhere else and talked about headaches? Have you been a guest on another podcast? Have you been interviewed locally? Every one of these is a signal that reinforces the same thing: you are an expert on this particular subject, in this particular area.

This is also where reviews matter more than most chiropractors give them credit for. Google and AI cannot interview your patients directly or ask around your community the way a person could in a Facebook group. Reviews, along with what people are saying about you on Reddit, in forums, and across the rest of the internet, are the closest thing AI and Google have to that kind of community reputation check. When someone is searching for “chiropractor near me,” they are rarely just asking who is closest. In most cases, what they are actually asking is who is the best chiropractor near me. Reviews are a major part of how Google and AI try to answer that.

The Rich Get Richer

This is also why momentum compounds in local search. Practices that already have a strong base of reviews, established content, and a track record of off-site mentions keep pulling further ahead, because Google and AI already recognize them as the authority. If you are starting from zero and trying to overtake someone who has spent years building that foundation, it is a genuinely difficult climb, and pretending otherwise does not help you.

This is also exactly why working with a chiropractic website company that understands authority-building, rather than one still leaning on keyword stuffing, matters so much. A website that is just packed with keywords and has no depth behind it is not laying a foundation you can build on. It is just noise.

Where to Actually Start

The starting point is not evaluating whether you need a new website company. The starting point is sitting down and asking yourself where you actually want to be known as an authority. Do not pick twenty things. Pick two or three, ideally things that are genuinely high value in your market, not obscure topics nobody is searching for.

Once you know what those two or three things are, that is where your time should go: your website content, your blog, your social media, your involvement in the community. Building consistent depth in a focused area is what separates an authority from a website that simply mentions a lot of services once and hopes for the best.
