One of the most common things I hear from chiropractors—especially when they first join my NEXT Step program—is this: “I want to build a referral-based practice.” It’s a great goal. In fact, I think it’s one of the best goals you can have as a chiropractor.

But while referrals are often seen as the “golden goose” of chiropractic marketing, most chiropractors don’t have a clear strategy to make it happen. They hope for it. They talk about it. But they don’t actually build it.

So let’s talk about it.

Why a Referral-Based Practice Is Worth Pursuing

Let’s be honest—marketing changes. What worked last year might not work this year. Platforms evolve. Ads get more expensive. Algorithms shift. That’s just the reality.

But what doesn’t change? People recommending people they trust.

A referral-based practice isn’t just about saving money on advertising. It’s about building a practice that’s more resilient, more sustainable, and more enjoyable to run. When people come in because they trust someone who already trusts you, that relationship starts off stronger. There’s less resistance. Less skepticism.

And frankly, it just feels better.

That said, building a referral-based practice doesn’t happen by accident. It takes time. It takes intentionality. And it takes the right strategy.


First Things First: You Have to Make Referrals Normal

One of the biggest mistakes chiropractors make is treating referrals like an afterthought.

If your patients have never heard you mention referrals, if you’ve never asked for one, if you’ve never shared how others have referred people to you, it’s not surprising when no one refers.

You have to create an environment in your practice where referrals are expected—where they’re normal. That includes reviews, sharing content online, and telling their friends. All of that has to be part of your practice culture.

When those things are normal, they happen more often. When they’re not, they happen less often. It’s that simple.


The Keys to Building a Referral-Based Chiropractic Practice

Here’s a quick breakdown of what it takes to make referrals a consistent part of your marketing strategy:

1. Make Referrals Normal
Talk about them. Ask for them. Celebrate them. Build a practice culture that encourages referrals without being weird or pushy.

2. Be Clear About Who You Help
Vague chiropractors don’t get referrals. If someone can’t clearly picture who they should send to you, they won’t send anyone. Focus on 2–3 core types of people or conditions that you want to be known for.

3. Make It Easy to Refer
Provide referral cards, easy links, or simple ways to tell others. The harder it is to refer you, the less likely it is to happen.

4. Provide Excellent Care
This should go without saying—but if you’re not delivering great results, none of the rest matters.

5. Build Trust
Don’t be shady. Don’t oversell. Don’t pressure. The more people trust you, the more likely they are to refer.

6. Have a Plan and Be Consistent
Referrals don’t happen in waves because of one conversation. They happen because you’ve been laying the groundwork for weeks, months, and even years.

7. Be a Referrer and Reviewer Yourself
If you want people to refer and review you, you should be doing the same for others. It builds goodwill—and reciprocity is real.


The Different Types of Referrals Chiropractors Can Get

Not all referrals come from your patients. Here are the five categories I break them down into:

1. Patient Referrals
These are the most common—and the easiest—because they come from people who’ve experienced your care directly.

2. Small Business & Professional Referrals
Don’t overlook people in your network just because they’re not patients. They might trust you, appreciate you, and happily refer others—even if they’ve never stepped foot in your office.

3. Medical Referrals
This could be MDs, DOs, PTs, massage therapists, or other providers. The key here is not to make it about you. Make it about how you can help them solve a problem—especially with patients they’re tired of trying to manage.

4. Community Referrals
These are second- or third-hand referrals based on your reputation. People refer to you because they’ve heard good things—even if they’ve never met you. This happens when your name comes up again and again as “the chiropractor people trust.”

5. Digital Referrals
Online reviews, social media shares, and content mentions are modern-day referrals. They may not carry as much weight as a personal recommendation, but they build credibility and visibility.


Why People Refer

There’s no single reason, but here are the most common ones:

  • They had a great experience
  • They trust you
  • You asked them to refer
  • You referred them or helped them
  • You’re viewed as an expert or authority
  • You made it easy and clear who to refer
  • They simply like you

It’s also worth pointing out: not everyone refers. That’s normal. Trying to get 100% of your patients to refer is a great way to be constantly disappointed. Focus on increasing your referral rate, not chasing perfection.


What About Non-Patient Referrals?

Non-patient referrals take longer. They’re harder to get. And they require a different mindset.

When you’re networking with small business owners, professionals, or medical providers, remember:

  • Start with people you already know
  • Get warm introductions whenever possible
  • Show up consistently in the community (BNI, chamber, charity events, etc.)
  • Focus on solving problems for them
  • Stay connected without being a pest
  • Know when to move on

A lot of the people you reach out to will never refer to you—and that’s okay. You’re planting seeds. Over time, you’ll find the people who do refer. And those relationships can be gold.


Who Should Chiropractors Be Networking With?

Here’s a general rule I like to follow:

1. Your Neighbors
Anyone within a reasonable distance of your office—regardless of what they do. If someone asks them about a chiropractor and they don’t know anything about you, that’s a missed opportunity.

2. People Aligned with Your Niche
If you want to help athletes, connect with coaches and gyms.
If you help moms and kids, connect with doulas, midwives, and parenting groups.
If you work with weekend warriors, connect with running clubs or CrossFit boxes.
Make sure the people in your niche know who you are and what you do.


Final Thoughts

A referral-based practice is worth chasing—but it’s not magic. It takes time, effort, and clarity. You have to build the culture, establish the connections, and consistently show up in ways that earn trust and visibility.
